What is Community Care Service Voucher (CCSV)?

CCSV is a scheme introduced by Social Welfare Department to allow eligible elderly choose community / home care services that suit their personal needs. Social Welfare Department will subsidize 60-95% of spending depending on the individuals’ affordability.

CCSV Authorised Service Providers

SCHSA’s EasyHome® Service has provided home care service to the elderly for over a decade.  In 2020, EasyHome® became an authorised service provider for CCSV.  CCSV recipient can use EasyHome® Service for home care, medical escort etc. This will help alleviate financial and caregiving pressure of many caregivers.

EasyHome® Service Coverage: Home-based Service, Medical Escort, Day-time Home Care, Personal Care Service

 

Home-based Service   Cleaning, Meal Preparation, Household Tidying, Shopping, Bills Payment.

Escort Service Medical Escort, Medicine Pick-up, Escort Elderly to go out.

Eligibility for CCSV

 

Person(s) assessed to be eligible and suitable for Community Care Service or Institutional Care Service who are on the Central Waitlist for Subsidized Long Term Care Service still waiting for and not having received subsidized community care service and/or institutional care service.

 

 

How can I apply for CCSV?

 

1. Applicants or Families need to apply for CCSV at Medical Social Service Department, or Integrated Family Service Centres or Elderly Service Units of their districts.

 

2. Social Worker or Officer will conduct assessment with the elderly applicant.

 

3. Those applicants assessed to have middle to severe level of impairment will receive an invitation letter from the Government to participate in the CCSV Scheme.

 

4. Applicants should fill out and return the relevant form.

 

5. Applicants shall receive the CCSV certificates as evidence that application is successful.
