1.	Monthly donation by credit card
	Please download and complete the Donation Form with your credit card details, and return to us by post or by fax on 2304 5193.

2.	Donation by autopay
	Please complete the Direct Debit Authorisation with donation details and return to us by post.

*	In order to reduce costs, the SCHSA will send out all monthly donation receipts for the preceding year by the end of April.



One-off donation

1.	Donation by cheque
	Please complete and return the Donation Form to us by post together with a cheque made payable to "Senior Citizen Home Safety Association".

2.	Donation by credit card
	Please complete the Donation Form with your credit card details and return to us by post or by fax on 2304 5193.

	Please call PPS Hotline at 18011 for registration and enter merchant code of SCHSA at 9195. Simply enter your contact number as the donor number.
