Tele-health Link is a new value added service to our Personal Emergency Link (PE Link) users started operation in January 2008. The service is piloted in collaboration with the Hospital Authority and local hospitals. It consists of 3 service components, Post-discharge Tele-health Advice, Health Advice Hotline and 24-hr Transmission of ePR (Electronic Patient Record).

It started operation in January 2008 in collaboration with United Christian Hospital(UCH) for PE Link users of 60 years old or above in Kowloon East:-
Kwun Tong, Lam Tin, Ngau Tau Kok, Kowloon Bay, Yau Tong, Tseung Kwan O, Sai Kung, Sau Mau Ping, Tiu Keng Leng, Hang Hau, Lei Yue Mun
Registered nurse will call high risk discharge patients from UCH at least twice after their discharge. Through which, appropriate health advice will be given with reference to their ePR. If so required, follow-up calls will be given by other staff and trained volunteers to keep track on their health condition.

It was started from April 2008 for all PE Link users in the territory. When they are sending to Accident and Emergency Dept in time of emergency, their extracts of ePR will be transmitted to hospital. Those information will be a quick reference for medical personnel and facilitating their preparation of treatment before patient¡¦s arrival by ambulance.

It is planned to launch in January 2009 for PE Link users of 60 years old or above in appointed region. Registered Nurse will give out appropriate health advice through the hotline. The detail of the service content will be announced afterward.
