A geriatric case manager will be assigned to every service user for taking care of his/her unique needs. Adopting the case management approach, together with the 24-hour PE Link Service, our geriatric case managers help seniors and their families with long-term care arrangements by conducting needs assessment, establishing care plan, suggesting service options, arranging the most suitable services and negotiating for the reasonable prices, so as to ensure the seniors receive the best possible in-home care.

Professional Case Management Service: Case Manager conducts care-planning assessment to identify problems and service needs, then basing on the assessment results and service users' preference to liaise and arrange the most suitable services, so as to ensure the service users receive the best possible in-home care. |
